• New Patient Diagnostic Evaluation (60 minutes): $350.00

• Established Patient Follow-Up (30 minutes): $185.00

• Established Patient Follow-Up with Therapy (55 minutes): $260.00

Administrative & Documentation Fees

• Medical Records Request (Digital/Paper): $25.00

• Professional Letters & Basic Forms: $50.00

• Emotional Support Animal (ESA) Evaluation & Letter: $75.00

• Disability Paperwork (Short Form): $100.00

• Disability Paperwork (Medium Length/FMLA): $150.00

• Disability Paperwork (Complex/Long Form): $250.00

Clinical Consultations & Communication

• Patient-Initiated Phone Calls (11–20 minutes): $50.00

• Patient-Initiated Phone Calls (21–30 minutes): $75.00

• Urgent/After-Hours Refill Request (Less than 3 business days notice): $25.00

Legal & Professional Services

• Court Hearing or Legal Appearance (4-hour minimum): $1,200.00

• Each additional hour of legal testimony/appearance: $300.00

Practice Protection Fees

• New Patient Late Cancellation or No-Show (Less than 48 business hours): $200.00

• Established Patient Late Cancellation or No-Show (Less than 48 business hours): $100.00

• Overdue Balance Fee (Greater than 30 days): $50.00

• Non-Sufficient Funds (NSF) or Chargeback Fee: $50.00
