I welcome referrals from therapists, primary care providers, and other clinicians seeking collaborative, high-quality psychiatric care for their patients. New clients can typically be scheduled within 2 weeks.

How to Refer a Client

To ensure the fastest onboarding process for your patient, please utilize one of the options below (listed in order of preference):

1. Online Screening (Fastest): Direct your client to this website and have them complete the New Client Screening Form located at the top of this page.

2. Secure HIPAA compliant Fax: Fax the formal referral and patient facesheet directly to 503-974-0922.

3. Contact Form: Complete the brief provider communication request form to to the left, and I will reach out to you directly.
