What is a Psychiatric-Mental Health Nurse Practitioner (PMHNP)?
A PMHNP is an Advanced Practice Registered Nurse who specializes in the holistic treatment of mental health conditions. My role at Cascadia combines the clinical rigor of medical science with the traditional nursing focus on the whole person. As a board-certified PMHNP, I am licensed to conduct comprehensive psychiatric evaluations, diagnose mental health conditions, and prescribe medications when they are indicated as part of a balanced treatment plan. I believe that being a Nurse Practitioner allows me to move beyond a simple diagnosis to understand how your biology, your environment, and your personal story all intersect.
What can I expect from our first session?
Our initial meeting is a foundational session designed to establish a clear, subjective account of your mental health journey. We will spend about 60 minutes exploring your medical and psychiatric history, your current challenges, and the goals you have for your wellness. This is not just a clinical interview; it is a collaborative dialogue where we discuss your medical background, your support systems, and any past experiences with treatment. By the end of this session, we will have the beginnings of a roadmap for your care, which may include further diagnostic exploration, lifestyle grounding strategies, or a discussion about medication options.
Do you accept my insurance?
Cascadia Psychiatry & Wellness is committed to making high quality care accessible and transparent. We accept most major insurance plans in Oregon, as well as the Oregon Health Plan (OHP), to ensure our services remain within reach for our community. For those who prefer to manage their care independently, we also offer a cash pay option and can provide services as an out of network provider.
Choosing an out of network or cash pay structure often allows for a higher level of privacy and more dedicated time for each session. If you choose this route, payment is due at the time of service, and we can provide a "Superbill" through the PracticeQ portal for you to submit to your insurance carrier for potential reimbursement. We recommend contacting your insurer directly to verify your specific mental health benefits and coverage options before our first appointment.
Do you offer telehealth or in-person appointments?
To establish a strong foundation for our therapeutic partnership, our initial appointment will be held in person. This face-to-face meeting allows us to connect more deeply as we begin our work together. Following our first session, you have the flexibility to choose the format that best fits your lifestyle. We offer a hybrid model for all future visits. Our telehealth option allows you to engage in your sessions from the privacy and comfort of your own home via a HIPAA-compliant video platform, removing the barriers of travel and time. To ensure a successful virtual session, I recommend a quiet, private space and a stable internet connection.
Is this practice inclusive of LGBTQIA+ and neurodivergent identities?
Absolutely. Cascadia Psychiatry & Wellness is built on the principles of affirmative care, providing a safe harbor for individuals across the full spectrum of identity. How do you approach medication management?
I view medication as one tool within a broader wellness toolkit. My philosophy is conservative, evidence based, and rooted in the art of prescribing. This approach is informed by years of managing complex pharmacological interventions in high acuity medical settings, where I learned the necessity of precision and clinical safety.
Our work is a collaboration. We will thoroughly discuss the risks and benefits of any intervention, and we will only move forward once you feel fully informed and comfortable. My goal is to find the most effective support at the lowest necessary dose, ensuring your long term stability and personal preferences remain at the center of every conversation.
How often will we meet, and how long does the process usually take?
The frequency of our appointments depends entirely on your current clinical needs and where you are in your wellness journey. Typically, we will meet more frequently at the beginning of our partnership or during periods when we are actively adjusting your treatment plan. If we are initiating a new medication or managing complex symptom changes, we may schedule follow-up appointments every one to two weeks to closely monitor your response, prioritize your safety, and ensure clinical precision.
Once your symptoms have stabilized and you feel grounded, our appointments will generally transition to a monthly routine. These regular check-ins allow us to collaborate on your ongoing progress, manage refills, and ensure your long-term stability remains on track.
The overall duration of the psychiatric process varies for every individual, as mental health is a dynamic landscape rather than a fixed timeline. Some patients utilize our partnership for short-term stabilization during a challenging life transition, while others benefit from ongoing, long-term medication management and therapeutic support. We will routinely evaluate your goals and progress together, ensuring that the pace of your care always respects your unique narrative and personal autonomy.
Do you coordinate care with my other doctors, providers, and therapists?
Yes. Cascadia Psychiatry & Wellness strongly believes that the most effective mental healthcare is collaborative and integrated. Because the mind and body are deeply interconnected, your psychiatric well being can be significantly influenced by underlying medical conditions, lifestyle factors, and concurrent therapeutic work. To ensure your safety and continuity of care, I routinely collaborate with primary care providers, medical specialists, and outside therapists.
To protect your privacy, this coordination requires your explicit consent. During your intake process or at any point during your treatment, we can complete a Release of Information (ROI) form through the secure patient portal. This authorizes me to share clinical notes, discuss treatment strategies, and align our goals with your other providers, ensuring your entire healthcare team is working in harmony to support your stability and narrative.
What is your policy on cancellations or missed appointments?
The time scheduled for your appointment is reserved exclusively for you. To maintain a sustainable practice and ensure that all patients have access to timely care, we require at least 48 business hours' notice for any cancellations or modifications to your appointment time.
If you miss an appointment or cancel with less than 48 business hours' notice, a late cancellation fee will be automatically charged to your card on file. This fee is $200.00 for an initial evaluation and $100.00 for an established follow up visit. Please note that insurance companies do not provide reimbursement for missed appointments or late cancellation fees; these charges are entirely the responsibility of the patient. If you need to reschedule, please utilize the secure patient portal as soon as possible to make the necessary changes.
How do I handle emergencies or crises outside of our scheduled appointments?
Cascadia Psychiatry & Wellness is a scheduled outpatient practice and does not provide 24-hour crisis coverage or emergency services. If you are experiencing a mental health emergency, severe distress, or a situation involving immediate safety concerns for yourself or others, you should not wait for a response from this office. Please utilize one of the following immediate options:
Call or text the 988 Suicide & Crisis Lifeline to speak with a trained counselor immediately.
Go directly to the nearest hospital emergency room, such as the Unity Center for Behavioral Health in Portland.
Call 911 and request a behavioral health crisis CIT officer if emergency assistance is required at your location.
For non-emergency matters that arise outside of our business hours, you are welcome to leave a secure message through the patient portal. These messages will be reviewed and addressed during our next scheduled office day. If you utilize a crisis resource between our sessions, we encourage you to share that information with us during our next scheduled appointment so that we can collaboratively update your care plan and ensure your ongoing stability.
